This form is used to confirm the claimant has accepted replacement item/s.

We (IVAA) have asked the Supplier to confirm you are satisfied with the replacement prior to invoicing to your insurance company.

By proceeding with this supplier, you acknowledge that you are happy for the Supplier to invoice the insurance company for the replacement item/s and this component of your claim is finalised.
